Unilateral Hyperlucency of the Lung: A Systematic Approach to Accurate Radiographic Interpretation

چکیده
cent hemithorax is seen at frontal chest radiography are often encountered. Because such an abnormality is related to various conditions, including radiographic artifacts, chest wall abnormalities, pulmonary vascular disorders with reduced perfusion, and lung parenchymal abnormalities with air trapping or compensatory overexpansion, a radiographic finding of increased radiolucency involving one hemithorax may give rise to difficulties in differential diagnosis. The accurate radiographic interpretation of unilateral hyperlucency therefore requires an awareness of the spectrum of these causes. The various conditions can be grouped on the basis of their underlying mechanisms and radiographic features, and according to our experience, an approach that reflects this is useful for their evaluation and differentiation. In this pictorial essay, we present the imaging spectrum of the conditions causing unilateral hyperlucent hemithorax, emphasizing the importance of a systematic approach to the radiographic interpretation of the reJ Korean Radiol Soc 2002;47:615-623

Basilar hyperlucency in a patient with emphysema due to hypocomplementemic urticarial vasculitis syndrome.
Disproportionate emphysematous involvement of the lung bases, compared with the apices, sometimes called basilar hyperlucency, is an unusual radiographic pattern that has been reported primarily in patients with alpha-1 antitrypsin deficiency, but also in individuals with emphysema caused by intravenous injection of methylphenidate. We present a patient with emphysema associated with hypocomple...

Swyer-James-MacLeod syndrome (SJMS), or unilateral hyperlucent lung syndrome, is a rare disease thought to be associated with post-infectious bronchiolitis obliterans (1). This disease was first described in 1953 by Swyer and James in a 6-year-old child who had been treated with pneumonectomy (2). MacLeod presented a unilateral hyperlucency series one year later. From this date on, the disease ...

A 16-year-old boy presented to our emergency room (ER) with cough, headache, myalgia, instant onset fever and runny nose. The patient was referred to our department due to density difference between each side on chest radiography (Figure 1). His complaints were concordant with influenza, however, the radiographic appearance indicated hyperlucency on the right side. Technical impropriety and pul...

Swyer-James syndrome (SJS) or Swyer-James-MacLeod syndrome is a rare disorder characterized by roentgenographical hyperlucency of one lung, lobe, or part of a lobe. As originally described, it shows a unilateral small lung with hyperlucency and air trapping on radiographs. It is postinfectious obliterative bronchiolitis due to childhood respiratory infection. In SJS, the involved lung or portio...
